Confidentiality
Who Has Access To My Disability Information?
Your file, containing your documentation, your Request
for Services form, and any other information concerning your case will
be kept COMPLETELY CONFIDENTIAL at the SDRC. ASIDE FROM SDRC STAFF,
NO ONE WILL HAVE ACCESS TO YOUR FILE UNLESS YOU GIVE WRITTEN PERMISSION
FOR RELEASE.
The SDRC staff can only discuss accommodation issues with
faculty after you give written permission on the Request for Services
form. Information regarding a disability will not appear on University
documents (i.e. transcripts, financial aid forms).
You may obtain a
copy of your documentation from your file, but you will be asked for
picture ID before it can be released to you.
