EXAM SIGN-UP FORM
SDRC has gone to using a three page form which provides
1). a white colored copy for SDRC
2). a yellow colored copy for faculty
3). a pink colored copy for the student
This system will not only provide accountability on everybody's part but serve as a reminder about test times and getting test sent to SDRC in a timely fashion.
All exam sheets must be completed and returned to SDRC at least "one week" before the exam is scheduled. The exam sheet should be filled out entirely by the student and the instructor. Note that a SPECIFIC TIME has to be entered in which to take the exam. Avoid putting down "anytime". Only those students who submit their exam sheet prior to their requested test date will be permitted to take their exam at SDRC.
- Please Remember: you must turn in your exam sign-up sheet at least "one week" before the date on which your exam is scheduled. Forms can be picked up at SDRC in the Exam room or outside the SDRC located in a wall pocket near the doors.
- Both you and your instructor must sign the sheet in order to take the exam at SDRC.
- You must take the exam at the scheduled date/time as agreed upon with your instructor. Notify SDRC of any changes and include the instructor's signature if the date/time has changed.
- You must complete all exams by 4:45pm (Mon-Fri). Exam proctors have permission to take any exams that are not completed at the time of closing.
Student responsibilities and Instuctor responsibilities:
- Student Responsibilities
- Bringing the COMPLETED white colored copy Exam sheet to SDRC at least 5 WORKING DAYS prior to the scheduled date/time of the exam.
- Taking the exam at the scheduled date/time agreed upon by the student and his/her instructor. Notify SDRC of any changes- include instructor's signature if the date/time is different from that indicated on the original exam sheet.
- Completing exams by 4:45pm (Mon-Fri). Exam proctors have permission to take any exams that are not completed at the time of closing.
- Leaving all non-exam materials (i.e. backpacks, cell phones, and pagers) in the SDRC front office unless the instructor has given written permission for the student to use exam aids (e.g. notes, books, calculators, dictionaries.)
- Complying with course exam guidelines and the University's Academic Honor Code.
- Instructor Responsibilities
- Assigning the date/time that the exam is to be taken. DO NOT sign a blank Exam Sign-Up Sheet. Fill in all of the information on the Instructor portion of the sheet.
- Listing any special examination aids (i.e.notes, books, calculators, dictionaries, tables/graphs, etc.) that may be used during the exam
- Keep the yellow colored Exam copy for faculty for your records.
- E-mailing (sdrc@admin.fsu.edu), faxing (644-7164), mailing (through campus mail/courier), or hand-delivering exams (108 Student Services Bldg) AT LEAST 48 HOURS BEFORE THE STUDENT IS SCHEDULED TO TAKE THE EXAM.
- SDRC Responsibilities
- Providing exam accommodations (i.e. extra time, readers, writers, quiet environment).
- Administering the exam on the date/time specified on the Exam Assistance Sheet.
- Proctoring/monitoring students as they take exams.
- Returning completed exams to respective instructors or departments by email or fax the same day unless the instructor chooses to pick up the exam from SDRC in 108 Student Services Bldg or allow the student to return it.
- Returning exams (via e-mail or fax) not taken by the student to the instructor one week after the test date.
